Secure your equipment carefully in a cardboard box with suitable packing material to ensure proper protection during shipping. It is not advisable to send equipment in an envelope.
Include this form and any applicable warranty documentation in the box.

GST applies to all rejected, out of warranty repairs and no-fault-found assessments. This excludes a return shipping cost of $14.30 Inc. GST where applicable.
